\u00bfC\u00f3mo mejorar el manejo cl\u00ednico de los pacientes con T-ALL\/LBL? Utilizando la informaci\u00f3n \u00f3mica y cl\u00ednica del paciente y los enfoques moleculares, nuestro objetivo es 1) identificar biomarcadores que puedan predecir la respuesta temprana a la terapia, permitiendo refinar la estratificaci\u00f3n terap\u00e9utica, y 2) identificar dianas terap\u00e9uticas que sean susceptibles de intervenci\u00f3n farmacol\u00f3gica y ofrezcan tratamientos alternativos y eficientes. <\/p>\n

Las neoplasias linfobl\u00e1sticas de c\u00e9lulas T (T-ALL\/LBL) son un tipo de c\u00e1ncer hematol\u00f3gico agresivo derivado de c\u00e9lulas T inmaduras, que afecta principalmente a ni\u00f1os y adolescentes. Los tratamientos actuales consisten en dosis altas de quimioterapia multifarmacol\u00f3gica seguida de trasplante de c\u00e9lulas madre hematopoy\u00e9ticas en pacientes de riesgo est\u00e1ndar alto. Las tasas de curaci\u00f3n alcanzan hasta el 90% entre los pacientes pedi\u00e1tricos, pero solo el 48% en los adultos, y se asocian con toxicidad aguda y a largo plazo. Adem\u00e1s, el 20-40% de los pacientes experimentan una reca\u00edda o no responden a los tratamientos de primera l\u00ednea, mostrando entonces un mal pron\u00f3stico con tasas de supervivencia inferiores al 10% y opciones terap\u00e9uticas muy limitadas. Es necesario abordar estas deficiencias en el manejo cl\u00ednico de los pacientes con T-ALL\/LBL. La falta de caracter\u00edsticas cl\u00ednicas o marcadores moleculares que puedan asignar con precisi\u00f3n a los pacientes con T-ALL\/LBL a un estadio de riesgo espec\u00edfico en el momento del diagn\u00f3stico explica, al menos en parte, la toxicidad asociada con el tratamiento.

Por otra parte, actualmente no se dispone de tratamientos dirigidos contra las principales alteraciones identificadas en la T-ALL\/LBL y, desde la aprobaci\u00f3n de la nelarabina en 2005, no se ha aprobado ning\u00fan inhibidor farmacol\u00f3gico para pacientes con T-ALL\/LBL en recidiva o resistentes al tratamiento (R\/R). Todo ello pone de manifiesto dos necesidades no cubiertas: 1) una estratificaci\u00f3n m\u00e1s refinada de los pacientes con T-ALL\/LBL en funci\u00f3n de su riesgo potencial, lo que permitir\u00eda mejorar las tasas de curaci\u00f3n y reducir la toxicidad; y 2) el desarrollo de tratamientos dirigidos contra las principales alteraciones que afectan a los pacientes con T-ALL\/LBL, lo que contribuir\u00eda a reducir las dosis de quimioterapia para evitar efectos tard\u00edos y ofrecer\u00eda una opci\u00f3n terap\u00e9utica para los pacientes R\/R. Nuestro grupo intenta contribuir a resolver estas dos cuestiones. Utilizando datos gen\u00f3micos, transcript\u00f3micos y cl\u00ednicos de los pacientes, algoritmos de priorizaci\u00f3n terap\u00e9utica basados en las caracter\u00edsticas \u00f3micas del paciente, junto con aproximaciones moleculares para validar nuestras hip\u00f3tesis, exploramos las alteraciones gen\u00e9ticas y moleculares en los pacientes para identificar marcadores moleculares que sean predictivos de una mala respuesta al tratamiento y que puedan obtenerse en el momento del diagn\u00f3stico. Adem\u00e1s, pretendemos identificar posibles dianas terap\u00e9uticas susceptibles de intervenci\u00f3n farmacol\u00f3gica. Como miembros del Instituto de Investigaci\u00f3n Sanitaria de la Fundaci\u00f3n Jim\u00e9nez D\u00edaz, nuestra investigaci\u00f3n cuenta con la participaci\u00f3n activa de nuestros compa\u00f1eros de los servicios de Hematolog\u00eda y Anatom\u00eda Patol\u00f3gica. Desde hace a\u00f1os, colaboramos estrechamente para asignar un significado cl\u00ednico a nuestros resultados moleculares. Con nuestra investigaci\u00f3n, esperamos contribuir a un mejor manejo cl\u00ednico de esta enfermedad, con una mejor identificaci\u00f3n de los pacientes de alto riesgo para guiar su tratamiento, y con una mayor eficacia de las terapias actuales en una fracci\u00f3n relevante de pacientes cuyo tratamiento dirigido es hoy en d\u00eda una necesidad no cubierta. En \u00faltima instancia, esto contribuir\u00eda a aumentar las tasas de curaci\u00f3n de los pacientes con T-ALL\/LBL. <\/span><\/p>\n
