El mal funcionamiento de esta enzima provoca la acumulaci\u00f3n de metabolitos t\u00f3xicos, como el propionil-CoA, que pueden da\u00f1ar \u00f3rganos vitales, especialmente el coraz\u00f3n. Aunque se conoce desde hace d\u00e9cadas que la acidemia propi\u00f3nica puede provocar arritmias y cardiomiopat\u00edas graves, los mecanismos que conducen a estas alteraciones a\u00fan no est\u00e1n claros.<\/p>\n<p>Para abordar esta cuesti\u00f3n, los equipos liderados por Eva Richard, Lourdes R. Desviat, en el Centro de Biolog\u00eda Molecular Severo Ochoa (CBM-CSIC-UAM), en colaboraci\u00f3n con Guofang Zhang, de\u00a0 Duke University (Durham, EE.UU.), utilizaron c\u00e9lulas madre pluripotentes inducidas derivadas de un paciente con la enfermedad para producir c\u00e9lulas de coraz\u00f3n (cardiomiocitos), y los compararon con c\u00e9lulas sanas. Mediante t\u00e9cnicas avanzadas de an\u00e1lisis metab\u00f3lico lograron rastrear en tiempo real el destino de distintos compuestos del metabolismo celular.<\/p>\n<p>Uno de los hallazgos clave fue la identificaci\u00f3n de una v\u00eda alternativa de defensa celular. Cuando el metabolito propionil-CoA se acumula, las c\u00e9lulas lo convierten en propionilcarnitina y propionato, este \u00faltimo siendo expulsado fuera de la c\u00e9lula como una especie de &#8220;v\u00e1lvula de presi\u00f3n&#8221;. Este mecanismo protector no se hab\u00eda descrito previamente y podr\u00eda ayudar a mitigar el da\u00f1o celular.<\/p>\n<p>Sin embargo, este sistema de defensa no es suficiente para evitar la disfunci\u00f3n card\u00edaca. El estudio, publicado en la revista <em>Cellular and Molecular Life Sciences<\/em>, revel\u00f3 que la acumulaci\u00f3n de propionil-CoA induce un cambio en el metabolismo energ\u00e9tico del coraz\u00f3n. Normalmente, los cardiomiocitos obtienen su energ\u00eda de la oxidaci\u00f3n de \u00e1cidos grasos, un proceso altamente eficiente. En cambio, en las c\u00e9lulas derivadas del paciente con acidemia propi\u00f3nica, se observ\u00f3 una transici\u00f3n hacia un metabolismo basado en glucosa, un combustible menos eficiente. Esta &#8220;reprogramaci\u00f3n metab\u00f3lica&#8221; podr\u00eda explicar la progresiva p\u00e9rdida de capacidad contr\u00e1ctil del coraz\u00f3n y su vulnerabilidad en estos pacientes.<\/p>\n<p>&#8220;Este cambio en el uso de combustibles energ\u00e9ticos podr\u00eda ser un factor clave en el deterioro progresivo del coraz\u00f3n en pacientes con acidemia propi\u00f3nica&#8221;, explica la Dra. Eva Richard, investigadora del CBM y una de las autoras principales del estudio. Adem\u00e1s, destaca la importancia de los modelos celulares derivados de pacientes: &#8220;Este tipo de enfoques personalizados nos permite observar directamente qu\u00e9 ocurre en un modelo celular humano, algo que no siempre se puede reproducir en modelos animales&#8221;.<\/p>\n<p>Estos hallazgos no solo mejoran la comprensi\u00f3n de la enfermedad, sino que tambi\u00e9n abren nuevas l\u00edneas de investigaci\u00f3n para el desarrollo de terapias dirigidas. Si se logra fortalecer el metabolismo mitocondrial o evitar el cambio de sustrato energ\u00e9tico, podr\u00edan dise\u00f1arse tratamientos m\u00e1s efectivos para prevenir o retrasar el da\u00f1o card\u00edaco en pacientes con acidemia propi\u00f3nica.<\/p>\n<p>El estudio ha sido posible gracias al apoyo de la <em>Propionic Acidemia Foundation<\/em> (EE.UU.), la <em>Fundaci\u00f3n Ram\u00f3n Areces<\/em> y el <em>Ministerio de Ciencia, Innovaci\u00f3n y Universidades.<\/em><\/p>\n<p>&nbsp;<\/p>\n<h3>Referencia<\/h3>\n<p>Metabolic flux analysis in hiPSC\u2011CMs reveals insights into cardiac dysfunction in propionic acidemia.
